Student Health Services
The Jack McFarland Health Services Building is conveniently
located
across from Cummings Dormitory and the Business and Aerospace
Building. The full time professional staff
consists of three board-certified physicians; two certified
physician assistants, two certified nurse practitioners and
six RNs,
all
of whom maintain certification in either ACLS or BCLS.
A prepaid student health fee covers the cost of basic office
visits.
There may be additional charges for
other professional services such
as laboratory tests, medications, injections, suturings,
and
EKGs. Payment of these fees is expected at the time of
service and may be paid by cash, check, Raider Funds,
MasterCard or
Visa.
Hepatitis/Meningitis
Immunization Health History Form
The
State of Tennessee mandates that MTSU provide information
concerning Hepatitis B infection and Meningococcal disease
to all students entering MTSU for the first time. All new
undergraduates and graduate students will need to electronically
sign a waiver form before they can register. This form is
located within PipelineMT.
Instructions
for locating/completing the form:
- Sign
in PipelineMT.
- Click
on the RaiderNet tab.
- Select
Student & Financial Aid.
- Select
Registration & Schedule.
- Select
Drop/Add Classes.
- Select
2008 Spring/Summer Term.
- The
form will display if the student is 18 years of age or
older
and the student will need to answer both questions and
click on the Submit button. Once completed, the student
will get
a confirmation page with a link "Return to Registration"
which takes them to the "Add Class" box to enter
call numbers.
- The
message informing a student to print a form from the web
will display if the student is under the age of 18. A parent
or legal guardian is required to sign the form and return
to Health Services. This form can be mailed or faxed to
615-898-5004.
Note:
A
hold is automatically set to prevent a student's registration
until the process is completed. Once completed, the hold automatically
removes and the student is clear to register. There
is no need to contact Health Services to remove the hold since
everything is automatic.

Health
Service Hours
The
Health Services Building is open Monday-Thursday, 8:00am
- 4:30pm
and Fridays 8:00am - 4:00pm during fall and spring semesters.
During summer and semester breaks open Monday-Friday 8:00am
- 4:00pm, and closed during university holidays. When Health
Services
is closed,
local
hospital
emergency
rooms
and some
walk-in
centers are available.

for MTSU students.
