Personal Statement Forms

The following links will lead you to the appropriate personal statement form for your classification. Please note that not all students are required to submit a personal statement in support of their application for admission. The personal statement is only required of students who do not meet standard admission requirements to the University.

The Personal Statement - Freshman Applicant is required of students who have no previous college experience, prior to high school graduation, and who do not meet guaranteed admission requirements . This personal statement is also required of all who hold a GED and have no previous college credit.

The Personal Statement - Transfer Applicant is required of students who have completed college coursework since high school graduation and who do not have a cumulative grade point average of 2.0 or greater on all attempted coursework.

The Personal Statement - Re-enrolling Applicant is required of all former MTSU students who have attended another college since their last enrollment at MTSU and whose cumulative grade point average is less than a 2.0.
