Forms and Applications
Upper Division Forms for the following Criminal Justice degree programs are available at the CJA Department Office, located in Jones Hall 223:
- Bachelor of Science in Criminal Justice Administration
- Bachelor of Science in Criminal Justice with a concentration in Law Enforcement
- Bachelor of Science in Criminal Justice with a concentration in Homeland Security
Students need to meet with their CJA advisor and minor advisor to complete the form and then file the completed form with the Graduate Analyst for the College of Education and Behavioral Science at the Records Office during the second semester of the sophomore year.
