Upper Division Forms
The rules for choosing the appropriate form are as follows:
1. The date of the form must be equal to or later than the year you first enrolled
at MTSU
2. The date of the form must be less than seven years before your proposed graduation
date
3. The date of the form must be a year that you were registered in classes at MTSU
Note: 2004-2007 forms are for students who wish to graduate under the 2004-2005 or
2005-2007 catalogs.
