Instructions: This form is to be submitted for requesting that 2 or more course sections be combined into a single offering (this should be requested prior to or immediately after the start of the semester. Requests must be submitted each semester. After submission, it may take up to 24 hours for the course mapping to be completed. This process cannot be easily undone. Be sure to consult with an IT Specialist at ext. 8189 if you have questions.
