Instructions: This form is to be submitted for requesting a non-enrolled user to be added to your course.  Non-enrolled users may be a faculty, student or guest who is not listed in the Banner Student roll for the class.  Please see the table below to note what must be submitted each semester.  After submission, it may take up to 24 hours for the user to be added.
