This form is for informational purposes only. The Financial Aid Office will mail the Assignment Sheet to the student before the semester(s) awarded. It will indicate the Department/Office the student is assigned. The student must contact the Department supervisor to determine that his/her work schedule for the semester is suitable.
The Financial Aid Office sends the agreement to the Department/Office supervisor, to be completed and returned. The agreement must be signed and dated by the student and supervisor. The agreement indicates how many hours the student is eligible to work each week, rate of pay, as well as the employment start and end date. The student's supervisor is responsible for returning this form to the Financial Aid Office.
IRS Form W-4 and INS Form I-9
Students must complete an IRS Form W-4 and an INS Form I-9. The I-9 form requires two forms of identification. MTSU requires the identification to be a copy of the student's Drivers License and Social Security Card. Both the W-4 and I-9 are mailed to the student upon assignment and must be returned to the Financial Aid Office. The I-9 form must be updated every three years. The W-4 form is only completed once.
