MMR Vaccine
The state of Tennessee requires students entering college, universities, and technical institutes with enrollment greater than 200 students to provide proof of two (2) doses of Measles, Mumps, and Rubella (MMR) vaccine from all full-time undergraduate and graduate students. TBR universities and colleges must obtain proof of full-time students' MMR immunization prior to the first day of classes. The MMR vaccine doses must have been administered on or after a student's first birthday or proof of immunity to measles, mumps, and rubella.
In compliance with this law, and in an attempt to maintain a safe and healthy campus environment, Middle Tennessee State University requires that all entering full-time students, born after 1956, furnish documented proof of having immunity to measles, mumps, and rubella or having been immunized with two doses of MMR vaccine on or after the first birthday unless contraindicated because of pregnancy, allergy to a vaccine component, or other valid medical reason(s).
The law states that immunizations are not required if they "conflict with individuals religious tenets and practices, affirmed under penalties of perjury." They are also not required if a qualified physician shall certify that administration of such immunization would be in any manner harmful to the child involved." (TCA Section 49-6-5001)
Acceptable documents are:
- The Certificate of Immunization completed and signed by a licensed doctor
- A copy of your shot record from your State Health Department
- A copy of an official immunization card
- A military immunization form or DD214 form will be accepted
- Official documentation from a prior college or university
- If you were previously enrolled at a Tennessee public or private college or university as a full-time student for at lease one semester after August 1, 2007, you may provide an official transcript as documentation of your MMR immunizations.
*Please disregard this form if you were born before 1957, if you are attending part-time, or if you graduated from a Tennesse public high school in 1999 or later.
YOU WILL NOT BE ALLOWED TO REGISTER FOR CLASSES FULL-TIME UNTIL AN ACCEPTABLE FORM IS ON FILE IN THE STUDENT HEALTH SERVICES OFFICE.

Hepatitis/Meningitis Immunization Health History Form
The State of Tennessee mandates that MTSU provide information concerning Hepatitis B infection and Meningococcal disease to all students entering MTSU for the first time. All new undergraduates and graduate students will need to electronically sign a waiver form before they can register. This form is located within PipelineMT.
Instructions for locating/completing the form:
- Sign in PipelineMT.
- Click on the RaiderNet tab.
- Click on the Student tab.
- Select Registration.
- Select Add or Drop Classes.
- Select 2009 Summer/Fall Term.
- The form will display if the student is 18 years of age or older and the student will need to answer both questions and click on the Submit button. Once completed, the student will get a confirmation page with a link "Return to Registration" which takes them to the "Add Class" box to enter call numbers.
- The message informing a student to print a form from the web will display if the student is under the age of 18. A parent or legal guardian is required to sign the form and return to Health Services. This form can be mailed or faxed to 615-898-5004.
Note: A hold is automatically set to prevent a student's registration until the process is completed. Once completed, the hold automatically removes and the student is clear to register. There is no need to contact Health Services to remove the hold since everything is automatic.
