WOMEN'S HEALTH HISTORY

already scheduled your annual exam, please

to fill out your paperwork. Once you login, click on
appointments, then answer the Women's Health questionnaire
attached to your appointment. If your paperwork is not complete
by the time of your appointment, you will be charged a $10
re-booking fee and will be asked to reschedule.
Student Health Services houses a clinic to serve the university's female student population. The Women's Center provides a wide range of medical services including annual examination with Pap smear. Various forms of contraception are also dispensed through our clinic. These services are provided by appointment only. Appointments can be made by clicking here to schedule online, calling 615-898-2988 or by stopping by the registration desk at the Health, Wellness, and Recreation Center.
