Create a Comment Form

FrontPage users follow the instructions for a FrontPage-specificform.

You may add a comment form to your web pages by using "" which is a CGI script.

"" is unique in that only home pages stored on frank can make use of the script. The comment form allows visitors to send comments or questions directly to you via a "fill-in-the-blank" form.

Include the following (preferably) in a new file named comment.html or, if you wish, in your .html file of choice:

NOTE: If you are using to administer a survey online, there is additional code to cut and paste into your web page to maintain data integrity.

		<FORM method="post"
		<INPUT type="hidden" name="recipient" value="
		<INPUT type="hidden" name="subject" value="
		<INPUT type="hidden" name="redirect" value="
		Web Page to Send User to after completing the form">
		<INPUT type="hidden" name="archive_filename" value="
		File you want the form data to be stored in">
		<INPUT type="hidden" name="archive_username" value="
		Frank account the data file will be located in">
		<INPUT type="hidden" name="sort" value="order:name1, name2,
		Your Name<BR>
		<INPUT size=30 maxlength=30 name="name1"><BR>
		Email address<BR>
		<INPUT size=30 maxlength=30 name="name2"><BR>
		Your Comment or Question<BR>
		<TEXTAREA rows=7 cols=60
		<INPUT type="submit" value="SendMe" name="submit">
		<INPUT type="reset" value="ClearForm" name="reset">

Line 5: Value is only the file name in your public_html; the complete path is not required.
Line 6: Value is the account name only (account name follows the tilde [~] in your web address).

If you want the data to be stored in a datafile, complete the value="..." field in lines 5 and 6. Before the data will be stored in the datafile, you must log into your frank account and run the script: "archive_setup."

frank $ archive_setup

As people fill out the form, their information will be appended to the file you designate for "archive_filename" (line 5). At any time, you may FTP that file to your computer.
