Reports of acts and incidents involving students may be referred for evaluation and possible disciplinary action by the Department of Public Safety, Residence Hall staff members, faculty, students, and other members of the University community.
To make a referral, you are required to submit written documentation outlining the specific facts about the incident including the names of those students involved. You should also include your contact information in the event that a staff member needs to follow up with you. If the incident involves academic misconduct, you should also indicate what grade you are recommending for the student. You may use the forms below or you may submit documentation in your own format. All documentation should be sent to campus PO box 17 to the attention of Judicial Affairs. You may also make referrals via email as long as all pertinent documentation is attached.
