2011-2012 Upper Division forms are not yet available online, check with the department
or with the college advisors. Course requirements change depending on which catalog year you are graduating under.
Speak with your advisor to make sure you are going under the applicable catalog requirements.

2007-2009 Upper Division Forms
Upper division forms should be filled out with the assistance of your faculty advisor.
Please check with the department to make sure that you have the most recent version
of your upper division form. Some students may be eligible to use an older version
of an upper division form - please check with your faculty advisor to see which form
is most applicable for your situation. Upper division forms should be filled out,
signed, and turned in to the Dean's office (Todd 231) along with an intent to graduate form three semesters before you are ready to graduate. For example: a student wishes to
graduate in Spring 2010; they should file their intent to graduate form and upper
division form by the end of Spring 2009
