Please Note: To use this online request form you must have the following:

Report number and/or name of involved party.
Scanned copy of your government-issued photo ID.

If you are unable to scan a copy of your photo ID to attach to this form, you can request a copy of the report in person at the Middle Tennessee State University Police Department, 1412 East Main Street, Murfreesboro, TN 37132 between 8:00 am – 4:30 pm Monday-Friday, or you may mail your request including the information below, along with a copy of your photo ID.

Media should continue to request reports through MTSU News and Media Relations. For more media information, please visit the MTSU News & Media Relations website.
