PARKING IS AT THE METERED SPOTS ON BOTH CHAMPION WAY AND MILITARY MEMORIAL.  THERE IS NO PARKING IN THE JONES LOT, YOU WILL GET A TICKET.  YOU CAN GO TO PARKING SERVICES AT 205 CITY VIEW DR AND PURCHASE A PARKING PLACARD.

Effective December 18th - Passport Agency is now handling routine service from 6 - 8 weeks and expediated service from 2 - 3 weeks.  It may take up to 3 weeks from the day you apply until your application is "in process".  Meaning that processing times start, after they have been entered into their system.

We will be closed for MEMORIAL DAY MAY 27, 2024.

                                                                  Resumption of Processing Times for Routine and Expedited Service

 Passport Services will resume processing times for routine and expedited services.  The processing times are as follows: 

  • Routine Service: up to 6 - 8 weeks from door-to-door, from date of application.
  • Expedited Service: up to 2 - 3 weeks, door-to-door, from date of application.  Expedited service costs an additional $60 and should be included with the passport fees at the time of application.  It is recommended that if you are expediting your application that you also use Priority Mail Express to return your new Passport Book.  Fee has increased for this service to $19.53
  • Currently we do not offer money orders so please bring a check for the U.S. Department of State.

Expedited Service is offered to customers at our Passport Acceptance Office.  Actual Passport Agencies will only handle expedited matters with a life or death emergency and are traveling within 3 business dasy.

Customers can check the status of their application online at travel.state.gov or by calling NPIC at 1-877-487-2778.

First time applicants must complete an Application for a U.S. Passport (Form DS-11) available at the MTSU Passport Services .  Note: Application should be completed prior to submitting.

Note: Youths 16  to  17 years old must apply in person with one parent.

Submit the following documents with the completed Form DS-11:

  1. Proof of United States Citizenship (only one of the following is required) 
    Note: All birth records must meet U.S. Department of State guidelines.
    1. Original State-issued Birth Certificate with raised, colored, or embossed seal
    2. Certified copy of State-issued birth certificate with raised, colored, or embossed seal
    3. Copies of birth certificates are not acceptable. Birth certificates may be obtained at the the CDC website. Note: Birth certificate abstracts from California and Texas are not acceptable.  The birth certificate must be a long form.  Birth records issued by a hospital are not accepted
      1. U.S. Citizens born abroad may submit one of the following:
        1. Original Certificate of Naturalization (Adults 18 or older)
        2. Original Form FS 240, Consular Report of Birth Abroad
        3. Original Form DS-1350, Certification of a Report of Birth Abroad
        4. Original Certificate of Citizenship
    4. Applicants may be required to submit the following in addition to proof of citizenship: Original name change documents (marriage license, divorce decree, adoption papers)
  1. Identity Verification
    1. Current state-issued driver's license Note: Out of state driver’s license requires additional ID
    2. Current state-issued ID Card
    3. Current Military ID

Note: If a full adult driver license is not available, multiple forms of ID must be submitted.  Please contact our office for other usable ID.

Minors aged 16 or 17 are not required to present photo ID if the accompanying parent has a valid current ID, such as current adult driver license, valid adult passport, or Military ID.  If the applicant has a valid ID it should be presented at the time of application.

  1. Photocopy of ID front and back (Copies can be made at MTSU Passport Service Department)
  2. Passport Photo: One recent passport photograph taken within the last six months.  Photo service is available by MTSU Passport Services, , this service costs $13.00 tax included. 
  1. Two Required Payments
    1. 1st payment: U.S.Department of State fees (Payable by check or money order only, and paid for by debit card)
    2. 2nd payment: MTSU fees (Payable by check, credit card, money order and cash.)
