Checklist for First Time Dual Enrollment Students

Apply for Admission to MTSU  Complete the online Dual Enrollment Application.
Submit the Application Fee There is a non-refundable $25 application fee (part of the Admissions application)
Submit Official High School Transcript

High School and College officials can use our secure Dynamic Form to send transcripts or we accept any electronic transcript services.

Submit Official ACT/SAT/Pre-ACT

Students who have not completed one of these tests must register for ACCUPLACER (MTSU Placement Test)

Apply for Dual Enrollment Grant  Must be completed each academic year.
Dual Enrollment Consent Form Download the form here and return to the Dual Enrollment office or your high school counselor
Meningitis/Hepatitis B Health History Form Form must be electronically completed prior to first semester of enrollment. Not a requirement for immunization; only an acknowledgement statement.
Submit Documentation for Eligibility Verification for Entitlements Act (EVEA) After receiving your MTSU ID number (M number), please follow the link to submit Documentation for Eligibility Verification for Entitlements Act (EVEA)
Submit College Transcripts (if applicable) Submit official transcripts of previous college credit to the MTSU Admissions Office, if you have taken classes from another institution.
Consortium Agreement (if applicable) If student is taking Dual Enrollment courses at more than one college/university in same semester, contact the Dual Enrollment office for paperwork.
