Checklist for First Time Dual Enrollment Students

Step Notes
Apply for Admission to MTSU  Complete the online Dual Enrollment Application.
Submit the Application Fee There is a non-refundable $25 application fee (part of the Admissions application)
Submit Official High School Transcript Send official high school transcript to ugadmissions@mtsu.edu.
Submit Official ACT/SAT MTSU School Code – 3994
Apply for Dual Enrollment Grant  Must be completed each year.
Dual Enrollment Consent Form  Form must be signed by student, parent, and guidance counselor/principal. Must be completed each year.
Submit Documentation for Eligibility Verification for Entitlements Act (EVEA)  Submit photo ID to evea@mtsu.edu. Click the link for more information.
Submit College Transcripts (if applicable) Submit official transcripts of previous college credit to the MTSU Admissions Office, if you have taken classes from another institution.
Consortium Agreement (if applicable) If student is taking Dual Enrollment courses at more than one college/university in same semester, contact the Dual Enrollment office for paperwork.
Submit Fee Discount Forms (if applicable) Complete a fee discount form - for dependents of MTSU or State of Tennessee employees.
