Checklist for First Time Dual Enrollment Students

Apply for Admission to MTSU  Complete the online Bachelor Degree Freshman Application. Click here
Submit the Application Fee There is a non-refundable $25 application fee (part of the Admissions application)  
Submit Official High School Transcript Send high school transcript to MTSU Office of Admissions, 1301 East Main Street, SSAC 120, Murfreesboro, TN 37132.  
Submit Official ACT/SAT/ACT Plan score MTSU School Code – 3994

* Hepatitis/Meningitis Health Form The hepatitis/meningitis form must be completed and returned to the Admissions office. Click here
* Dual Enrollment Consent/Verification Form  Form must be signed by student, parent, and guidance counselor/principal. Click here
* Complete Partners in Education Form (P.I.E.) Form required to release student information to chosen individuals.  Click here
Submit Documentation for Eligibility Verification for Entitlements Act (EVEA)  Submit photo ID to Click the link foror more information. Click here
Submit College Transcripts (if applicable) Submit official transcripts of previous college credit to the MTSU Admissions Office, if you have taken classes from another institution.
Consortium Agreement (if applicable) If student is taking Dual Enrollment courses at more than one college/university in same semester, contact the Dual Enrollment office for paperwork.  
Submit Fee Discount Forms (if applicable) Complete a fee discount form - for dependents of MTSU/TBR or State of Tennessee employees. Click here
Register for Courses Must be fully admitted as an MTSU student to register for classes Click here
Pay any fee balance and confirm enrollment Check Dual Enrollment website under Payment tab Click here
